Clinical Competencies exam

- Mulakan dengan bismillah. Yakin!
- WIPE! Wash ur hand. Introduce urself. Position the patient (Abdomen 1 pillow, cardio 45degree on bed). Expose the patient (take off shirt).
- Jangan gelabah x tentu hala. U know ur stuffs VERY WELL. insyaAllah,u'll be fine.
- Ask the patient's consent. "you have pain anywhere? stop me if u have any".
- Biar patient adapt dulu dengan kesejukan tangan anda (alkohol + cuak = sejuk melampau)
- Jangan lupa kneel down sebelum examine abdomen patient ye cik leen. Penting ni sebab nak make sure sudut wrist anda tu betul.
- Untuk cardio + respiratory exam, sila make sure anda auscultate menggunakan both bell and diapragm.
- Neuro exam plak, logik je ni macam da blaja dalam anatomy. InsyaAllah ok.
- Reflex! Berusaha! infrapatella tendon tau. owh ye,reinforcement manoeuvre (clench teeth) + Jendrassik manoeuvre (interlock finger and try to pull them apart)
- Tadi kak anis cakap examiner suka mintak test 7th and 5th nerve (sensory + motor-masseter,pterygoid,temporalis). Jangan lupa! for 7th, "Wrinkle ur forehead-bilateral cortical representation" "Close ur eyes" "Blow out ur cheeks" "Show me ur teeth" - itu sahaja ye cik leen. jangan lebih2 plak.
Jom revise associated symptoms plak!
GIT : ingat dari atas ke bawah.
- Nausea / vomiting
- Dysphalgia - difficulty in swallowing
- Heartburn / acid regurgitation
- Loss of appetite/ Weight loss
- Abdominal distension
- GI bleeding -haematemesis,melaena
- Jaundice
- Diarrhoea
- Constipation
CARDIOVASCULAR
- Chest pain!
- Dyspnoea - shortness of breath
- Proxysmal nocturnal dyspnoea - wake up at night gasping for air
- Orthopnoea - many pillows used during sleep
- Palpitation - aware of ur own heartbeat
- Intermittent claudication - cramp-like pain (leg)
- Ankle swelling - right ventricular failure
- Anxiety
- Sweating
- Syncope / faint
- Fatigue
RESPIRATORY
- Chest pain
- Cough
- Sputum / phlegm - volume/per day, colour, smell
- Wheezing
- Hemoptysis - cough up blood
- Fever
- Night sweat
- Weight loss/ allergies/ hoarseness
NEUROLOGICAL
- Headache (SRCOPDSARA)
- Faints
- Fits
- Visual (diplopia) /hearing/smell/sensation disturbances
- Dizziness
- Weakness
- Tremor (resting/intention/postural)
- Slurring speech
- Orientation
